Instructor Renewal Application Form
Please download Renewal for CDP and submit both documents. Only pay the fee for instructor renewal. You must attach proof of NCCDP Alzheimer's and Dementia training. Minimum of one training per year for renewal. This can be sign in sheets, brochure or certificates for those dates. You additionally need 10 ceus every two years. However if you have conducted the entire 7 hour training, it counts as double the hours.
Effective May 2005, any instructor
who wishes to be approved, must complete
the Train the Trainer NCCDP Course.
The NCCDP will review each application
on a case by case basis. The Train
the Trainer course is a one day course.
The course includes all the materials
needed to present a comprehensive Alzheimer's
and Dementia Seminar to long term care
professionals. The course includes:
Curriculum Manual/Overhead copies,
Power Point Presentation, Handout Notebook,
Text Books, and Communication Video.
The schedule for 2006 and 2007 Train
the Trainer is posted on the web site.

Applicant Checklist:
In order to complete the application
process, the applicant must meet
the following criteria:
1) Certified or licensed in a health
care profession
2) Bachelor’s degree or higher from
an accredited college or university
3) A minimum of 5 years experience
in a health related field/profession
within the past 7 years
4) Minimum 3 years experience as a
geriatric health educator and/or an
instructor in geriatric health care
