Please click on the RENEW ONLINE link to renew your Instructor renewal. The CDP renewal is now included in the online form.

If you do not wish to pay online, please download the INSTRUCTOR RENEWAL FORM and mail in the renewal forms. You only pay the fee for the renewal of your instructor certification. The CDP renewal fee is included. If you are a CDCM you will need to complete the CDCM renewal form. If you have taught the NCCDP Alzheimer's Disease and Dementia Care seminar, it will count double towards your 10 CE or 10 CEU's required every two years to renew. If you have not taught the NCCDP seminar, you will need to complete 10 contact hours or CEU's in any health care related topic.

Effective May 2005, any instructor who wishes to be approved, must complete the Train the Trainer NCCDP Course. The NCCDP will review each application on a case by case basis. The Train the Trainer course is a one day course. The course includes all the materials needed to present a comprehensive Alzheimer's and Dementia Seminar to long term care professionals. The course includes: Curriculum Manual/Overhead copies, Power Point Presentation, Handout Notebook, and Text Books. The schedule for Train the Trainer is posted on the web site. Please contact us if you have any questions uing the CONTACT US FORM or 1 877 729 5191. Please note you must complete both the CDP application and the instructor application. All forms must be submitted together.

Applicant Checklist:
In order to complete the application process, the applicant must meet the following criteria:

1) Certified or licensed in a health care profession
2) Bachelor’s degree or higher or RN, LPN, LVN from an accredited college or university
3) A minimum of 5 years experience in a health related field/profession within the past 7 years
4) Minimum 3 years experience as a geriatric health educator and/or an instructor in geriatric health care
