If you do not wish to pay online, please download the INSTRUCTOR RENEWAL FORM and mail in the renewal forms. You only pay the fee for the renewal of your instructor certification. The CDP renewal fee is included. If you are a CDCM you will need to complete the CDCM renewal form. If you have taught the NCCDP Alzheimer's Disease and Dementia Care seminar, it will count double towards your 10 CE or 10 CEU's required every two years to renew. If you have not taught the NCCDP seminar, you will need to complete 10 contact hours or CEU's in any healthcare-related topic. If you are fully retired, no contact hours or CE's are required.

You will not be able to renew online if you are 90 days past your renewal date. You will need to discontinue the use of NCCDP materials and you will need to retake the NCCDP CADDCT course. An email renewal reminder was emailed to you two months before your renewal date. A renewal card was also mailed to you. If you have any questions please contact firstname.lastname@example.org

YOU ARE WELCOME TO ALSO USE FEDEX OR ANOTHER SHIPPING SERVICE BUT INCLUDE THE PREPAID SHIPPING LABEL AND APPROPRIATE SHIPPING CONTAINERS OR ENVELOPE FOR THAT SERVICE. PLEASE ADDRESS YOUR RETURN ENVELOPE WITH YOUR SHIP TO ADDRESS. YOUR CERTIFICATION WILL NOT BE TO YOU MAILED IF THIS IS NOT PROVIDED.
If you are mailing in your CADDCT renewal application we recommend that you use either UPS or FedEx or the US Post OFFICE signed receipt. Please allow 4 to 6 weeks to process and mail you your new certification.
Please be sure to add the credentials of CADDCT and CDP after your name.
CADDCT stands for Certified Alzheimer's disease Dementia Care Trainer. CDP stands for Certified Dementia Practitioner.
If you are also a CDCM and a CADDCT you will also need to renew your CDCM online.
If you have taught the Alzheimer's disease and Dementia Care seminar, the 8 contact hours will count double towards your required 10 CEU's or 10 Contact hours required every two years. Be sure to also give yourself a certificate of attendance each time you present the seminar. If you have not taught the seminar you will need 10 CEUs or 10 contact hours in any healthcare-related topic from any source. We will not ask for proof of the hours unless you are selected for an audit. Please save (for two years) your contact hours and CEUs in the event of an audit. If you are fully retired, no contact hours or CE's are required.
We recommend using NCCDP Online Learning powered by NetCE - our preferred partner for Continuing Education Credits. Go to NetCE to obtain your CEUs.

The curriculum is updated every two years on an even year. In 2016 the curriculum was updated. Did you order the new curriculum?
Please go to the instructor only part of the web site to download the PowerPoint curriculum and also go to bulk orders to order a new student handout notebook and an instructor notebook. The notebooks will be shipped to you. All notebooks are to be ordered through the instructor only part of the web site unless you have received permission to use another printing source. You may only order new products if your certification is current.
Post all seminars on the calendar.

Always fax or scan your sign-in sheets within 7 days of the class. Our fax is 19738602244 and our email is email@example.com. We must have the person's email address on the sign-in sheet.

Never give the corporate discount form to anyone other than the Administrator. The employee if paying for their CDP application the fee is $135.00 pp and the employee does not get a discount. The discount only comes into play IF the Administrator is paying for a group of employee applications and the fee is then $35.00 pp and there is a $30.00 group shipping fee.
If you are presenting at a state or national conference, please provide the association discount form to each attendee in your seminar. You must sign the form. They have 30 days to mail in their CDP application once the class is completed. The fee is $35.00. The Association will NEVER collect the CDP application nor fees.
