Certified Dementia Support Group Facilitator™ CDSGF™ Renewal Application
To begin the online renewal process please complete the form below. Your next renewal date will automatically pull up as you enter your first name, last name, certification number and email address. If any of this information has changed and you enter a different last name (as an example) this program will not find you in the database.
If your name and/or email address has changed since your initial certification or last renewal - you will need to update your records at NCCDP. Please complete the name change/email change form.
In order to renew your CDSGF for this renewal period you are required to have completed 10 ceus or 10 contact hours that are health care related from any source over a two year period. NCCDP also offers an online learning.
Your CDSGF renewal date is noted on your certification.
You are to renew your certification on or before your certification expiration date. Regardless of paying early, the renewal date will not change. For example, your certification reads January 21, 2018 to January 21, 2020. Your renewal date will always be January 21st and the year the certification expires.
A renewal notice will be emailed and mailed to you two months prior to the renewal date. There is a $35.00 late fee if you renew after your renewal date.
You have until midnight of your renewal date to either renew online or mail in your renewal that is post marked no later than the expiration date.
