I understand that I may download the Tool Kit and the staff in-service, pre-test, post-test, and handouts with the express understanding that the tool kit and in-service documents may only be used at your facility, LTC setting, association, government agency, Physicians office, NP office, rehab center, hospital, adult daycare, hospice agency, place of worship, or home care agency to train staff, volunteers, support group participants, and family. You may not change, modify, copy, or distribute this information in any format, and the information provided in the tool kit and in-service documents is the intellectual property of the NCCDP. Copies may be made to distribute to your in-service as mentioned above. The Tool Kit and in-services documents may not be sold or distributed in any format. At no time may these documents be given as handouts if a participant is paying to come to a seminar. This can only be taught live and cannot be loaded on a company website or any other teaching medium other than offered live.
